Advance Health Care Directive (AHCD) (page 31) designates your choice of a legal decision-maker in your stead and specifies how you want to be cared for.

BENEFICIARIES

An easy, direct way to provide for your best-loved charities is to change the beneficiary designation on an IRA or life insurance policy to the names of those organizations. When these assets are given to individuals (instead of charities), they are considered as taxable income to the recipients.
